Knowledge and the use of a disease activity score in axial spondyloarthritis obviously  is important. Physicians  around the world almost always rely on this when making treatment choices for patients.  However, this critical measure, isn't always followed as strictly as it should be.

          The focus was on patients with active axial spondyloarthritis who weren't seeing changes in therapy.  Even though their disease activity level was high (ASDAS of 2. 1 or above), treatments remained unchanged.  In some cases  problems were identified by the doctors that were caused by the  ASDAS.

             Patient complications were a frequent issue. Almost always, concerns over treatment adjustments played a huge part.  Plus white coats arising logistical challenges and limited treatment options.  Lots appeared to think this was all worth the hassle. These hurdles made treatment more difficult.  Unfortunately, all this hampers the change from a basic goal to a more targeted goal.

                    On the other hand, several factors are positive. Enhancers include patients' detailed information about the disease. They should know a lot about axSpA. This would help, even though it might not make up for scarcity of evidence. .  More good news is there is solid communication between patients and doctors. This is  essential for a Shared decision making.

            In several cases lack of a proper infrastructure in clinics  was a massive issue. Increasing access to outpatient clinics could definitely help,  so would more support.

                           But doctors need to get with the times and step up . It is important for them to be aware of what is going on.

             Dig deeper, however. We see that the  findings are more complicated than just helping a group of people with one condition.  it is part of a wider picture.  Understanding all the difficulties and benefits is crucial. This is vital since it can help guide the future.
