A new tool helps doctors estimate how long a patient will stay in the intensive care unit after oral free flap surgery, especially when no early tracheostomy is performed.

The instrument is called a nomogram, a chart that combines several patient factors into one score.

Researchers gathered data from hundreds of surgeries and identified key variables that influence ICU length of stay, such as:

  • Age
  • Weight
  • Underlying health conditions
  • Details about the flap procedure

Using statistical models, they created a formula that translates these variables into a predicted duration of ICU care.

To ensure the tool’s reliability, the team tested it on a separate group of patients who had not been used to build the model. Results showed that the nomogram accurately identified those likely to need longer ICU support, allowing teams to plan resources and prepare families for a potentially extended recovery.

Beyond the numbers, the study highlights that avoiding early tracheostomy can still lead to prolonged ICU stays in certain patients, underscoring the need for careful pre‑operative assessment.

Clinicians can now use this chart at bedside to discuss expected ICU times with patients and adjust postoperative care plans accordingly.
